L4 Lead Practitioner in Adult Care
The Lead Practitioner in Adult Care will inspire team members to make positive differences in someone’s life. They will be recognised as a lead practitioner sustaining and promoting a values-based approach at an operational level. The role includes mentoring and coaching others responsible for care delivery in a range of settings.
With this apprenticeship, you will be managing a team of frontline workers supporting individuals with care requirements. You will be leading a team and inspiring them to make positive differences to individual’s lives.
You will be in a position where you can exercise autonomy and accountability while leading and supporting others with complying with expected standards and behaviours. This is a varied role and can be found within a number of different care environments.
Tasks and responsibilities
- Statutory frameworks, standards, guidance and Codes of Practice which underpin practice in relation to the safe delivery of services
- Theories underpinning own practice and competence relevant to the job role
- Principles of assessment and outcome based practice
- Principles of risk management
Dignity and human rights
- How to contribute to, promote and maintain a culture which ensures dignity is at the centre of practice
- Effective communication and solutions to overcoming barriers
- Legal and ethical frameworks in relation to confidentiality and sharing information
- Range of technologies to enhance communication
- Legislation, national and local solutions for the safeguarding of adults and children including reporting requirements
Health and wellbeing
- Models of monitoring, reporting and responding to changes in health and wellbeing
- Range of holistic solutions to promote and maintain health and wellbeing using person centred approaches
- Importance of effective partnerships, inter-agency, joint and integrated working
- Goals and aspirations that support own professional development and how to access available opportunities
Who is this course for?
The apprenticeship is ideal for existing staff who wish to progress, retrain or upskill or those. This qualification is useful for learners who provide direct support and those working in sectors including:
- social care
Units / Modules
- Level 4 Diploma in Adult Care.
- Apprentices without Level 2 English and maths will need to achieve this level prior to completion of their Apprenticeship.
- For those with an education, health and care plan or a legacy statement the apprenticeships English and maths minimum requirement is Entry Level 3 and British Sign Language qualification are an alternative to English qualifications for whom this is their primary language.
- Lead Practitioner in Adult Care Level 4 Standard.

Final decision and grade confirmed as Fail, Pass or Distinction.
You should be at least 18 years old
You will need to be working in the sector with the ability to demonstrate competence in both knowledge and skills
You will need to be working at least 30 hours per week.
